How do I Get Started?
Please call or email our office if you have questions or concerns prior to starting the intake process. At that time, we will review our offerings and discuss current scheduling availability. Prior to scheduling, you should request a referral for evaluation and treatment concerns from your primary care provider be faxed to our office at (855) 370-5735. Once a referral is in place, please click here to complete forms online. In addition, you will need to submit a copy of your identification card and both sides of your health insurance card. If your child has previously received treatment, copies of previous evaluation, progress notes, and discharge summary is also beneficial. Once all required documents are submitted, you may contact the office to schedule your initial appointment.
WHERE WILL MY CHILD BE SEEN?
Appointments are available in our office or via telepractice. At this time, services are no longer available in your child's home, childcare center, or private school.
DO YOU ACCEPT INSURANCE?
Yes! We are in-network with United Healthcare, Tricare, and Anthem Blue Cross Blue Shield (including the CU Exclusive Health Plan). If we are not a participating provider with your insurance company, you may pay for your services in advance and request reimbursement from your insurance company. We will provide you with appropriate invoices and documentation to support your request.
What Will my Insurance Cover?
Each insurance company and benefit plan is different. You should contact your insurance company prior to initiating speech-language or feeding therapy services. Please see our Understanding Your Insurance Form which includes questions to ask your insurance company to best understand your insurance benefits and potential out of pocket expenses. Please note when you call that covered benefits often have exclusions or require medical necessity.
